At Grace Health& Wellness, we specialize in designing individual fitness plans to suit each Client's need. We believe in order to be whole you must address your spiritual, emotional, and physical barriers. Below is an example of a fitness plan that we created for one of our clients. 

FITNESS

Client Name: Annise

Age: 45

Occupation: She has been teaching for over 20 years, and has four adult children

CW:205

BMI: 31.2

Health Category: Obesity = BMI of 30 or greater

 

Stages of Change Model used for Assessment (TTM):

Contemplation/Preparation: Annise is aware of the consequences of her actions and is ready to change. She was eager to begin her wellness plan.

 

PAR-Q Results (Exercise Readiness Questionnaire):The client has no preexisting medical conditions and was cleared to begin a wellness plan. She is aware that untreated obesity can lead to other chronic conditions, such as diabetes or even hypertension. The client was an exercise enthusiast in her twenties and is ready to reclaim her health.

Barriers and Lifestyle: Annise has an all-or-nothing approach to life. She has invested over 20 years in educating schoolchildren. She is a divorced single mother who raised all her children, so she is used to pouring into others. However, she has neglected her own health. So her energy has been low and that can be a barrier. We want help redirect that energy and funnel it back into her. 

 

Time can also be a barrier.  She works long days but is usually home by 5:00 pm and ready to relax before looking over assignments. Our goal is to help her prioritize health and overall wellness, while ensuring that we don't change her routine so drastically that she cannot stick with it. 

Fitness Assessment Results:

  • Aerobic Fitness: 1 mile Rockport Walk test in 12 minutes. VO2 Max of 28.2 (fair)

  • Muscular Strength: 12 curl repetitions (fair) 2 push-ups (very poor)

  • Flexibility: sit and reach test, 4.0 inches (average)

Focus:

​We will focus on motivational interviewing to determine a strong why for completing the fitness plan. We will also develop a sense of community within the program and accountability partners in her everyday life. We encourage all clients to find at least one person within their immediate circle to join them on their journey​

Goals:

The client wants to lose weight and reach a healthy BMI range of 15.5-24.9. She wants a low-impact, sustainable program. She is a stickler to her schedule, so she wants to be able to incorporate new fitness habits instead of those that would drastically change her routine.

SMART Goals: We created two smart goals for Annise. Smart goals must be specific, measurable, attainable, relevant, and have a time frame. The first goal is to increase overall aerobic activity, so for the next six weeks, Annise will be on a high- to middle-intensity walking plan. She can walk on her lunch break with a co-worker or in her neighborhood once she returns home. She will complete this activity three times per week.

We also want to increase her strength. Twice a week, she will lift medium weights. As she increases her strength, we will add weight to how much she lifts. She wants to be able to do push-ups as well. Her starting push-up is two. We will begin with modified push-ups, ten per day over the next 3 weeks, five days a week. Weeks 4-6, she will complete twenty push-ups, five days a week. They can be split up to ten at lunch and ten in the afternoon when she returns home.

Insight Walking Plan:

We encouraged the client to always listen to her body. Ensure that her body is warm before attempting any exercise. Pay attention to any uncomfortable pains and don't push herself to the point of injury. She is a former exercise enthusiast but it important to start with a managable workout in order to maintain throughout the 6 weeks and beyond.

Insight Weight Plan:

Weights should be be 8 to 10lbs. Two sets with 12-15 reps. One set of dumbbell curls and one set of triceps curls. Light stretching is recommended after each workout. Workout plans to be revisited and modified at the conclusion of the 6-weeks. â€‹
